Ground Zero Analysis, Inc. <br /> SITE SAFETY PLAN <br /> EMERGENCY MEDICAL TREATMENT <br /> All GZA field personnel must be CPR and first aid trained. At a minimum 'personnel who inhale <br /> hazardous materials must be removed to fresh air. Personnel who are contaminated with hazardous <br /> materials by contact should rinse the area of contact for a minimum of 15 minutes. The site safety <br /> officer or her/his alternate will determine if injured personnel require further emergency medical <br /> attention. <br /> Unusual first aid procedures: <br /> Location of first aid kit: All GZA vehicles carry a first aid kit. <br /> Nearest emergency medical facility: St. Joseph's Oak Park Hospital <br /> Directions from work site: West on Miner Avenue to California. North on California. <br /> Hospital will be on right. <br /> Attach a map which shows the route from the work site to the nearest emergency medical facility. <br /> ❑ Check when done. <br /> SPECIAL EMERGENCY DIRECTIONS <br /> PAGE 10 <br />
