The drum of drained used oil filters did not have an accumulation start date. Facilities that generate <br /> up to one ton of used oil filters per year may store the filters up to one year. Since it cannot be <br /> determined how long this waste has been on site, immediately recycle these filters under a bill of lading <br /> and submit a copy of the bill of lading to the EHD by 3-17-11. Class 2 Violation. <br /> 27. The 55 gallon drum of used oil was left open. All hazardous waste containers shall be closed at all <br /> times except when adding or removing waste. Immediately close this container and ensure all <br /> hazardous waste containers are closed when not adding or removing waste. Repeat Class 2 Violation. <br /> 50. The open 55 gallon drum of used oil has an accumulation start date of 6-30-09. This waste has been <br /> on site greater than 180 days. Facilities who generate less than 1000 kg of hazardous waste per month <br /> and do not exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 days. <br /> Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and <br /> submit a copy of the manifest to the EHD by 3-17-11. Class 2 Violation. <br /> 52. An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. A form is provided that can be <br /> used for this purpose. Submit proof of correction to the EHD by 3-17-11. Repeat Class 2 Violation. <br /> 79. A bundle of universal waste lamps are left outside leaning against wall near the dumpster. On the <br /> ground outside next to the dumpster lay the broken remains of the universal waste lamp(s). Facility must <br /> store universal waste lamps in a closed container and in a manner that prevents breakage and <br /> release to the environment. Facility must immediately clean and containerize any broken lamps or <br /> releases to the environment. Correct immediately by managing broken or unbroken universal waste <br /> lamps correctly. Class 2 Violation. <br /> 83. An unmarked/unlabeled bundle of universal waste lamps are left outside leaning against wall near <br /> the dumpster. Facility must mark universal waste to identify type of universal waste. Correct immediately <br /> labeling these as "universal waste lamps".
