The following is an itemized list of hazardous materials violations that have not been <br /> addressed for PERFECTION AUTO PAINT as of April18, 2017. <br /> Open violations from March 01, 2017 inspection <br /> Violation#5 - Failed to complete and/or submit hazardous materials inventory for all reportable materi Is on <br /> site. <br /> Reportable amounts of acetone, paint and clear coat were found on site that have not been reported. Any ma erial <br /> that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into <br /> the California Environmental Reporting System (CERS) at http-Hcers.calepa.ca.gov/, add the materials to the <br /> hazardous material inventory, and submit to the EHD for approval. <br /> Violation#6 - Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergen y <br /> response equipment. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel <br /> Employee training records were not available at the time of inspection. The business plan shall include provi ions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. A site <br /> map was not completed or submitted as part of the business plan. <br /> Violation #12 - Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The em rgency <br /> response plans and procedures, the inventory of information required by this article, and the site map requireby <br /> this section shall be readily available to personnel of the business or the unified program facility with responsi ilities <br /> for emergency response or training pursuant to this section.
