3. Reporting and Notification Requirements <br /> a. All reports and information requested by the <br /> Department shall satisfy the signatory requirements in <br /> item II.H. The waste minimization certifications as <br /> required in item II.J shall be signed in accordance <br /> with item II.H. <br /> b. Annual Report <br /> The owner or operator shall prepare and submit one <br /> copy of an annual report to the Department and one <br /> copy to the appropriate Regional Water Quality Control ) <br /> Board by March 1 of each year, beginning, <br /> March 1, 1990. The annual report shall cover facility <br /> activities during the previous calendar year and shall <br /> include the following information: <br /> (1) The EPA identification number, name, and address <br /> of the facility; <br /> (2) The calendar year covered by the report; <br /> I <br /> (3) Updated closure cost estimate for the facility; <br /> (4) The EPA identification number of each hazardous <br /> waste generator from which the facility received <br /> a hazardous waste during the year; for imported <br /> shipment, the report shall give the name and <br /> address of the foreign generator; <br /> (5) The description, quantity, and method of <br /> treatment, storage, and/or disposal of each <br /> hazardous waste the facility received during the <br /> year, listed by the EPA identification number of <br /> each generator; and <br /> (6) Environmental monitoring data in accordance with <br /> Section 67195, Title 22, CCR. <br /> 0. Closure p"� • <br /> 'f m <br /> 1. Closure Plan and Amendment of Plan <br /> a. The owner or operator shall 'comply,; vit j the closur_it: <br /> plan as described in the approved Operation Pian. <br /> b. The owner or operator may amend his closure plan at <br /> any time during the active life of the facility. (The <br /> active life of the facility is that period during <br /> which wastes are periodically received. ) The owner or <br /> operator shall propose to amend his plan any time <br /> changes in operating plans or facility design affect <br /> the closure plan or whenever there is a change in <br /> expected year of closure. <br /> -24- <br />
