Facility personnel shall take part in an annual review of the initial training received. The owner/operator shall maintain the following documents and records at the facility: a)the job title for each position at the facility related to hazardous waste management, and the name of each employee filling each job; b)written job description for each position listed in a) above; c)written description of the type and amount of both introductory and continuing training that will be given to each person filing positions listed in a)above; d) records that document that the training has been given to and completed by facility personnel. Training records on current personnel shall be kept until closure of the facility. Training records on former employees shall be kept for at least three years from the date the employee last worked at the facility. Immediately provide employees with initial or refresher hazardous waste training. Within 30 days(July 16, 2011) provide this office with a copy of the roster and the syllabus which meet the requirements of 66265.16 CCR (California Code of Regulations). 60, 61, &62. Manifest numbers 005526378JJK, 005963205JJK, and 005965072JJK were found without a signed off copy from the destination facility. Manifest number 006887345JJK was not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. By July 16, 2011, submit a copy of the signed off copy of the manifest or a copy of the exception report submitted to DTSC. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corre ted, including supporting documentation, by July 16, 2011. Vii. ALL EHD STAFF TIME ASS IATED WITH FAILING TO COMPLY BY ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
