The owner/operator shall maintain the <br /> following documents and records at the facility: <br /> a)the job title for each position at the facility related to hazardous waste management, and the name of each employee filling <br /> each job; <br /> b)written job description for each position listed in a) above; <br /> c)written description of the type and amount of both introductory and continuing training that will be given to each person <br /> filing positions listed in a)above; <br /> d) records that document that the training has been given to and completed by facility personnel. <br /> Training records on current personnel shall be kept until closure of the facility. Training records on former employees shall be <br /> kept for at least three years from the date the employee last worked at the facility. Immediately provide employees with initial <br /> or refresher hazardous waste training. Within 30 days(July 16, 2011) provide this office with a copy of the roster and the <br /> syllabus which meet the requirements of 66265.16 CCR (California Code of Regulations). <br /> 60, 61, &62. Manifest numbers 005526378JJK, 005963205JJK, and 005965072JJK were found without a signed off copy <br /> from the destination facility. Manifest number 006887345JJK was not found on site. Hazardous waste generators shall <br /> retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for <br /> review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the <br /> facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial <br /> transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the <br /> status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date <br /> that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible <br /> copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste <br /> and the results of those efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. By July 16, 2011, <br /> submit a copy of the signed off copy of the manifest or a copy of the exception report submitted to DTSC. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corre ted, including supporting documentation, by July 16, 2011. <br /> Vii. <br /> ALL EHD STAFF TIME ASS IATED WITH FAILING TO COMPLY BY ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPEC 1014 AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD I ct R edBy: Title: <br /> EHD 2 003 Rev 08/10/10 CONTINUATION FORM <br />
