1 <br /> E KMR, <br /> 1 ' <br /> Mr James L Barton <br />' January 26, 2005 <br /> Page 6 <br />' effective at decreasing concentrations of dissolved TPHg and benzene in Unit B, Unit C and <br /> Unit D beneath the site <br />' PuEge Water Waste <br /> Non-hazardous purge water generated during fourth quarter 2004 groundwater monitoring and <br />' sampling activities was transported to Evergreen Oil Company in Newark, California, in <br /> December 2004 <br />' Monitoring Well MW-1 <br /> On August, 26 2004, ENSR notified the RWQCB, Stone Bros and GHH Engineering, <br /> Incorporated (GHH) that the following observations were made by an ENSR technician on <br /> August 26, 2004 <br /> 1 The well lid for groundwater monitoring well MW-1 was broken and that approximately 20% <br /> of the lid was missing <br /> 2 Fresh asphalt had been forced into the void space surrounding the upper portion of the well <br /> casing in the well box <br /> 3 The well cap was found sitting inside the well box <br /> 4 The top of the well casing did not appear to be damaged but the well cap was not on the top <br /> of the casing and there was a significant amount of asphalt inside the well box and around <br /> the opening of the well casing The opinion of our field staff is that asphalt and or other <br /> debris may have fallen into the open well casing <br /> 5 An asphalt contractor Cal Coating, (Site foreman Jose Montero (209) 667-8443 ) was onsite <br /> repaving the parking lot around the Macaroni Grill <br /> ENSR's actions upon notification <br /> 1 ENSR re-attached the locking well cap to MW-1 and placed a barricade over the well <br /> 2 ENSR contacted Gary Hall of GHH on August 26, 2004, approximately 9 30 a m to convey <br /> the situation to him <br /> 3 ENSR exchanged the well box lid for MWA with the well box lid for MW-3 (located in a <br /> planter) to temporarily limit a trip hazard and possible additional damage to MW-1 <br /> Between August 26, 2004 and September 1, 2004, the RWQCB requested that Stone Bros <br /> submit a work plan and schedule to assess potentiai impacts to MW-1 <br /> Monitoring Well MW-1 Assessment <br /> On August 31, 2004, at the request of the RWQCB, ENSR collected one groundwater grab <br /> sample from MW-1 and submitted the sample to CLS for analysis of TPHg, total petroleum <br /> hydrocarbons as diesel (TPHd) and motor oil by EPA Method 8015 Modified and VOCs by EPA <br /> Method 8260B Except for motor oil reported at 2 4 micrograms per liter (pg/L), analytes were <br /> J 1Prolects%0694015098-2931WorkinglReportsW98 4th Qtr 04-QMR doc <br /> ' Celebrating 35 Years of Excellence rn Environmental Services <br />
