An inspection report was issued identifying information to be <br />submitted to bring this site into compliance. A Return_ to Compliance Letter dated April 17, 2009 was previous <br />sent requesting this facility to return to compliance. The requested information was required to be submitted by <br />November 24, 2008 and still has not been received resulting in a non-compliant status for this facility. <br />Failure to correct the violations will result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($105).
