The contingency plan was not available at the time of inspection. A copy of the contingency plan and all revisions to the plan shall be maintained at the facility at all times. The contingency plan must include: 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non -sudden release of hazardous waste to air, soil or surface water at the facility; 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and State and local emergency response teams to coordinate emergency services; 3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and all others in order in which they will assume responsibilities; 4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; 5. Evacuation routes for facility personnel where there is a possibility of evacuation; 6. Current telephone number for State Office of Emergency Services. If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to fulfill the requirements of this chapter. This is a minor violation.
