Backus [EH]; Frank Girardi [EH]
Subject: CIWMB Unregistered Hauler Substitution Form 204
Good Morning, Carla,
I got your fax with a copy of the phone message from Adreana at CIWMB regarding the CIWMB Unregistered
Hauler Substitution Form 204. In response, I called and spoke to Keith Cambridge with the CIWMB Waste Tire Program.
The consensus of our discussion was that due to the amount of paperwork at the state level, they do not want you, as a
government entity, to send copies of this form each month to document where your tires at the Maintenance yard are
coming from (roadside pick up). They do want you to continue sending the CTL for transporting the tires to the landfill,
however.
As the Lead Enforcement Agency with the CIWMB Waste Tire Program, we have required that Public Works-
Road Maintenance document waste tires entering the yard at the end of each day. We ask that you continue to do so and
retain copies for three years to view during routine Waste Tire Survey Inspections conducted by our department. Until .
further notice these forms are not to be mailed to the COMB.
Should you have any questions, please feel free to contact me at (209) 468-9966
Jenni Frase Ruiz
San Joaquin County Environmental Health Department
