oar SAMAQUIN <br />—COUNTY <br />Environmental Health Department <br />Time In: 8:00 am <br />Time Out: 8:23 am <br />Mobile Food Facility Complaint Inspection Report <br />Name of Facility: LONCHERIA CHAPALA #7F53451 Date: 11/17/2017 <br />Address: 1717 S UNION ST, STOCKTON 95206 <br />Owner/Operator: HERNANDEZ, MANUEL Telephone: (209) 298-5416 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: C00045061 <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />C00045061 states: "COMPLAINANT ALLEGES ON 11/3/2017 COMPLAINANT ORDERED A BURRITO AND WHEN <br />THEY OPENED THE TIN FOIL THERE WAS A COCKROACH. PLEASE CALL COMPLAINANT WITH FINDINGS." <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7: <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION -- TEMP ° F --COMMENTS <br />No Temperature Data Collected <br />NOTES <br />Owner is aware of the complaint. <br />Upon thorough inspection of the vehicle's interior and exterior, no cockroaches (live or dead / at any life cycle) were observed. <br />Truck is in clean sanitary condition. <br />Ownership change occurred on 11-7-17 (change of owner inspection). Current owner has been operating the truck for about 8 <br />months. <br />OK to permit not given until refrigeration retro-fit occurs (per owner, this will occur next week). <br />Owner will pay for permit today and change over in system-refrigeration will be checked when work is completed. <br />OK to operate once payment is made. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
