Manifest numbers 002570518SKS (1/25/11), 002844077SKS (8/30/11), <br />002663763SKS (11/23/11), 000038492DAT (12/8/11), and 002952770SKS (1/10/12) were not found on <br />site. Copies of three of the manifests were located on the transporter's website, but TSDF signed copies <br />could not be found. Hazardous waste generators shall retain copies of all manifests for three years or <br />until a copy signed off by the destination facility is received. The signed off copy shall be retained and <br />made readily available for review for three years. If the generator did not receive a copy of the manifest <br />with the handwritten signature of the owner or operator of the facility to which the generator's waste was <br />submitted within 35 days of the date the waste was accepted by the initial transporter, the generator <br />shall contact the transporter and/or the owner or operator of the designated facility to determine the <br />status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within <br />45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an <br />Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the <br />generator explaining the efforts to locate the hazardous waste and the results of those efforts. <br />Exception reports shall be submitted to. <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. <br />By September 19, 2013, submit to the EHD a copy of the signed off copy of the manifests or a copy of <br />the exception report submitted to DTSC. <br />REPEAT VIOLATION <br />610. A blue metal bin of drained used oil filters was observed without an accumulation start date. <br />According to the records on site, the bin was emptied on August 13, 2013. Used oil filters must be <br />stored in a closed, rainproof container, labeled as "drained used oil filters, and marked with an <br />accumulation start date. Immediately label the date on the container and ensure that all used oil filters <br />are managed in this way. <br />REPEAT VIOLATION <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by September 19, <br />2013 along with a statement documenting the corrective actions that have been or will be taken for each <br />violation and any supporting paperwork. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY IME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Stacy Rivera (209) 468-3440 <br />Received B <br />Title: � <br />Q <br />ViW <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
