This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br />of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately <br />contact DTSC and reactivate your EPA ID number and submit evidence to the Environmental Health <br />Department (EHD) by December 30, 2010. A form has been provided. <br />Class 2 repeat violations <br />20. Three 55 -gallon drums of waste oil were observed at the facility. One drum was labeled "hazardous <br />waste" only. Two drums were not labeled. All hazardous waste containers shall be marked with the <br />following information: <br />1. the words "Hazardous Waste" <br />2. name and address of generator <br />3. hazardous properties <br />4. physical state <br />5. composition (contents) <br />6. accumulation start date <br />Immediately label these containers and ensure that all containers are marked with all the required <br />information. Since it cannot be determined how long the three 55 -gallon drums have been on site, <br />immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and <br />submit a copy of the manifest to the EHD by December 30, 2010. <br />23. One 55 -gallon drum of used oil filters was at the facility and labeled "drained used oil filters" only. <br />Used oil filters must be stored in a closed, rainproof container, labeled as "drained used oil filters," and <br />marked with an accumulation start date. Immediately add the accumulation start date to the label and <br />ensure that all used oil filters are managed in this way. <br />23. One 55 -gallon drum of used oil filters was at the facility and labeled "drained used oil filters" only. . <br />Facilities that generate up to one ton of used oil filters per year may store the filters up to one year. <br />Immediately recycle these filters under a bill of lading and submit a copy of the bill of lading to the EHD <br />by December 30, 2010, <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD s <br />Received By <br />4,� <br />Title: <br />IIS c a pewsl ae w <br />23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
