CONTINUATION FORM <br />FICIAL INSPECTION REPORT <br />Page: 1 <br />Date: 10/19/07 <br />Facility Address: 4945 E. Waterloo Rd. <br />Program: H <br />Hazardous Waste Complaint Inspection — C00026757 <br />A complaint was received alleging F&H Construction dug five holes 15 feet wide and 8-10 feet <br />deep in the northwest corner of the property and that they buried between 15-20 55 -gallon <br />drums of paint, oil, and paint thinner. This supposedly happened six years ago. <br />A site visit was made to investigate this complaint this day and to perform a routine hazardous <br />waste inspection. Spoke with Kevin Smith, Shop Maintenance who showed me the northwest <br />corner of the property. This area currently has storage lockers on it and no evidence of any <br />current excavations. Kevin has been with the company for 4'/2 years and doesn't recall <br />anything there. Eugene George, Secretary Treasurer, said that the area used to be their <br />hazardous waste storage area.
