Do you conduct treatment in the tank/system under authorization of Permit - <br />by -rule, Conditional Authorization, or Conditional Exemption? <br />F Yes- Your tank/system is a "Tiered Permitting System Tank" (see TIP -5, <br />appendix 1) <br />F No- Go To "B" <br />B. What type of waste is accumulated in the tank/system? <br />F RCRA- Go to "C" <br />IF Non-RCRA- Go to "E" <br />C. Does the whole facility generate less than 1000 kg (_270 gal. or 2200 lb.) of <br />RCRA wastes in a month OR is the waste held in the tank otherwise exempt <br />from RCRA regulation? <br />F Yes- Go to "D" <br />F No- Go to "F" <br />D. Was the tank/system installed or operational before July 1, 1991? <br />F Yes- Your tank/system is a " Existing tank, RCRA waste accumulation, RCRA <br />exempt/)/RCRA small quantity generator (SQG)" (see TIP -5, appendix 4) <br />F No- Your tank/system is a " New tank, RCRA waste accumulation, RCRA <br />exempt/RCRA small quantity generator (SQG)" (see TIP -5, appendix 5) <br />E. Was the tank/system installed or operational before July 1, 1991? <br />F Yes- Your tank/system is a " Existing tank, non-RCRA waste accumulation only" <br />(see TIP -5, appendix 2) <br />F No- Your tank/system is a " New tank, non-RCRA waste accumulation only" (see <br />TIP -5, appendix 3) <br />F. Was the tank/system installed or operational before July 14, 1986? <br />F Yes- Your tank/system is a "Existing tank, RCRA waste accumulation only" (see <br />TIP -5, appendix 6) <br />F No- Your tank/system is a " New tank, RCRA waste accumulation only" (see <br />TIP -5, appendix 7) <br />
