This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br /> of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately <br /> contact DTSC and reactivate your EPA ID number and submit evidence to the EHD by November 19, <br /> 2010. A form has been provided. <br /> 27. One 55-gallon drum of oily liquid (1/4 full) was not closed. All hazardous waste containers shall be <br /> closed at all times except when adding or removing waste. Immediately close these containers and <br /> ensure all hazardous waste containers are closed when not adding or removing waste. <br /> 84. Three 96 inch long fluorescent lamps were observed in the trash area (not in dumpster). Dispose of <br /> these lamps as specified in Title 22 California Code of Regulations. Send a copy of the disposal receipt <br /> to the Environmental Health Department by November 19, 2010. <br /> Summary of Violations: Class 2: 27, 84. <br /> Notes: <br /> 1. The facility has two 55-gallon drums which were each approximately 1/8 full of liquid. One was <br /> labelled "hazardous waste" only and one was unlabeled. The operator placed all of the liquid in the <br /> drum labelled "hazardous waste" and completely filled out the label. <br /> 2. Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by November 19, 2010. <br /> 3. Inspector gave County Small Business Hazardous Waste Facility brochure to operator. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THI F�AfflLUY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Ins Received B : Title: <br /> P <br /> EHD 2 -003 Rev 08/10/10 CONTINUATION FORM <br />
