(HSC 25191) Name: Mike Eliason Title: Card1 ock Manager Signature Date: 3/9/2016 Summary Statement to accompany the Return to Compliance statement for the Small Quantity Hazardous Waste Generator Inspection Report for 1512 Weiss way in Escalon, Dated 2/1/2016 and Amended 2/9/2016 Items 403 and 605 both dealt with an absorbent rag found in an open and unlabeled container. The rag was removed and placed in a properly labelled and covered container. The incident was immediately addressed with our divers and additional training was done in a drivers training meeting on March 4,2016 .
