Yosemite Ave, Manteca <br /> The permit application to expose product lines, dispenser pans, and sumps for the exploration <br /> of a leak has been approved with the following conditions: <br /> 1. A complete description of work performed must be submitted within 10 days of <br /> completing the repair. <br /> 2. If any piping is to be repaired or replaced Environmental Health must be notified so <br /> that an inspection can be arranged. Soil sampling may be necessary per Environmental <br /> Health assessment. <br /> 3. Manufacturers specification sheets including instructions for the repair must be <br /> submitted prior to beginning the repair. Per Environmental Health assessment of <br /> findings, new plans and a permit application may be required to be submitted. <br /> 4. A 30-minute line pressure test must be performed on the primary and secondary piping <br /> of all exposed lines and witnessed by Environmental Health prior to backfilling. <br /> 5. If sumps or dispenser pans are changed a 24-hour lake test will be required. <br /> 6. After completion of work, a line tightness test must be performed by a certified third <br /> party technician on all exposed piping. Results must be sent to Environmental Health <br /> within 10 days of completing the repair. <br /> 7. If concrete is broken near the underground storage tank a tank integrity test performed <br /> by a licensed tank tester after the cement is poured. Results must be received by this <br /> office within 10 days of completing the repair. <br /> 8. Notify Environmental Health of all third party test dates. <br /> 9. Notify Environmental Health once work is complete. A final inspection may be <br /> required. <br /> 10.
