The State Water Resource Control Board (SWRCB) <br /> will invoice you directly for activities performed under the auspices of the Local UST <br /> Oversight Program. The goal of the site evaluation performed by Public Health Services <br /> Environmental Health Division (PHS/EHD) is to certify closures of underground tank leakage <br /> sites. Closure certification occurs only after mitigation activities are completed. <br /> Y" <br /> In addition, the "Regional Board Staff Recommendations For Initial Evaluation and <br /> Investigation of Underground Tanks" and "Appendix A" reporting format and the LUFT <br /> Manual shall be utilized as guidance documents in the preparation of work plans and/or <br /> reports. <br /> Recently, work has occurred at this site with respect to excavation (floor/wail) and stockpile <br /> soil sampling for petroleum hydrocarbon contamination. PHS/EHD has not approved or <br /> ? witnessed this work and as a result may reject the analytical data generated. All proposed <br /> future characterization and remediation for this site must be submitted to PHS/EHD for <br /> approval prior to the commencement of related work and advance notice (PHS/EHD <br /> recommends 48 hours) for site inspections must be agreed upon by PHS/EHD, contractors <br /> and responsible parties. <br /> Please submit all original laboratory related reports, batch quality assurance, quality control <br /> + (QA/QC) data, and documentation from soil analyses presented to this office for the June <br /> + 12, 1995 excavation and stockpile sampling event.
