This pre-approval is specific to the activities as outlined in the June 5, 1998 proposal by Apex Envirotech,Inc. (Apex) for the replacement of an off-site domestic well which has leen impacted by contamination from the Moore Petroleum site. With the following provisions, the total cost pre-approved as eligible for reimbursement is $ 7,050, see the table below for a breakdown of costs. The Central Valley Regional Water Quality Control Board has concurred with the determination by San Joaquin County Environmental Health Division (SJCEHD) that no further action related to the underground storage tank release is required. Prior to site closure, it is necessary to install a new well to replace the impacted domestic well. Please be aware that this pre-approval does not constitute a decision on reimbursement: all reasonable and necessary corrective action costs for work directed and approved by the Regional Board will be eligible for reimbursement per the terms of your Letter of Commitment at costs consistent with those pre-approved in this letter. COST PRE-APPROVAL BREAKDOWN Task Amount Pre-Approved Comments Drill Well $5,980 Drill and case 6-inch well to 360 feet deep. Cost includes all materials and permitting. Install Well $1,070 Pull pump out of existing well and re-install in new well, trench 20 feet from old well to new well, install water lines and pvc conduit. TOTAL PRE-APPROVED $ 7,050 • The actual costs and scope of work performed must be consistent with the pre-approval for it to remain valid. + The work products must be acceptable to the Regional Board. • If a different scope of work becomes necessary, then you must request pre-approval of costs on the new scope of work. • Although I have referred to the Apex proposal in my pre-approval above, please be aware that you will be entering into a private contract: the State of California cannot compel you to sign any specific contract.
