HORIZON ENVIRONMENTAL INC. <br /> �• �� Specialists in Site Assessment Remedial Testing Design and Operation <br /> .lune 20, 2005 <br /> Ms Vicki McCartney fa; ewt �v <br /> Environmental Health Department G 1 X 3/ r- <br /> San Joaquin County '0& 1 <br /> 3 04 East Weber Avenue, Third Floor C-4 4 <br /> Stockton, California 95202-2708 <br /> Subject Replacement of Monitoring Well MW-1 <br /> Quik Stop Station No 076 <br /> 1030 South Olive Street, Stockton, California <br /> Ms McCartney <br /> Horizon Environmental Inc (Horizon) has prepared this report documenting the replacement <br /> of monitoring well MWA at the subject site located in San Joaquin County as shown on <br /> Figure 1 (attached) In a letter dated August 23, 2004, the San Joaquin County Environmental <br /> Health Department (EDH) directed replacement of monitoring well MW-1 (as shown on <br /> attached Figure 2) because the integrity of the well was apparently compromised due to <br /> cement grout entering the casing during placement of the sanitary seal Horizon arranged for <br /> Gregg Drilling & Testing, Inc to replace the monitoring well Horizon submitted a work plan <br /> with well permit application to the EDH in a latter dated May 2, 2005 The EDH granted <br /> approval and issued well permit SR# 0042429 on May 23, 2005 EDH correspondence and <br /> permit are attached to this letter <br /> On June 1, 2005, Horizon oversaw Gregg destroy and re-install monitoring well MW-1 Prior <br /> to well destruction, depth to water in monitoring well MW-1 was 64 49 feet below top-of- <br /> casing Monitoring well MW-1 was originally installed as a 2-inch diameter casing in an 8- <br /> inch diameter boring to 75 feet below surface grade (bsg) Monitoring well MW-1 was <br /> drilled out with 8-inch diameter augers to a depth of 75 feet The boring was then over-drilled <br /> with 10-inch augers to a depth of 80 feet The replacement well was installed in the 10-inch <br /> boring as a 2-inch diameter FVC well with 0 020-inch machined-slotted casing set over the <br /> interval 60 to 80 feet bsg The wellhead was covered with a traffic-rated, 14-inch diameter <br /> well box set at '/4-inch above surface grade Construction details for monitoring well MW-1 <br /> are shown on the attached well construction diagram <br /> Four separate samples of the stockpile of well construction materials and soil were collected <br /> for disposal characterization The soil samples were submitted, with chain-of-custody (COC) <br /> documentation, to Kiff Analytical LLC (Kiff, State of California certification #2236), <br /> combined into one composite sample and analyzed for total petroleum hydrocarbons as <br /> gasoline (TPHg), benzene, toluene, ethylbenzene, total xylenes (BTEX) and the fuel <br /> oxygenate methyl-t-butyl ether (MTBE) by EPA Method 8260B No analytes were detected <br /> in the composite soil sample The analytical report is attached to this letter The stockpile <br /> 4970 Windplay Drive #C5 • El Dorado Hills CA 95762 • (916)939-2170 • FAX(916)939-2172 <br /> PO Box 5283 • Bakersfield CA 93388 • (661)589-8389 - FAX(661)589-1456 <br />
