I went back to the Second Quarter 2014 <br /> report and updated the reasons for denial. I provided them below, with the previous reasons related to my <br /> miscalculation crossed out. Also, the Dual Phase Extraction Remediation Report-Third Quarter 2014 has been denied <br /> today after review. Please have denials corrected and reuploaded at your earliest convenience. <br /> -Dual Phase Extraction Remediation Report-Second Quarter 2014- <br /> Page 1, section 2.1: MW-19 is listed twice and incorrectly listed with DPE-2, DPE-3. <br /> Page 1, section 2.1: DPE-2, DPE-3 screen interval is incorrect. <br /> Page 4, section 2.3.2: upper end of hydrocarbon concentration range does not agree with <br /> Table 2 for this quarter. Note:this also appears on page 7, bullet point 2. <br /> Page 5, Historical Mass Removal and Implications: equivalent gallons of 2,256 pounds is miscalculated. <br /> 1 <br />
