GROUND WATER EXTRACTION WELL ACCESS AGREEMENT <br /> MANASSERO PROPERTY <br /> 4415 Pacific Avenue, Stockton, California <br /> Advanced GeoEnvironmental,Inc.(AGE)is hereby granted permission to install one groundwater extraction <br /> well, for the purpose of assessment and/or remediation of soil and ground water as the result of gasoline <br /> contamination from former underground storage tanks(USTs)at 4405 Pacific Avenue,Stockton,California <br /> ("PACIFIC CAR WASH"), on the property identified as 4415 Pacific Avenue, Stockton, California <br /> ("MANASERRO PROPERTY").All investigation/remediation will be performed under a directive/approval <br /> by the San Joaquin County Environmental Health Department and the Central Valley Regional Water Quality <br /> Control Board <br /> The scope of work includes advancing one, ten-inch diameter soil bore-hole to approximately 50 feet <br /> utilizing a truck-mounted drill rig.Upon completion of drilling,a six-inch diameter,perforated well casing <br /> will be placed through the center of the bore-hole,and well construction materials(i.e. sand,bentonite and <br /> grout)added to fill the void space between the well casing and the bore-hole.A traffic rated,bolt-secured, <br /> flush mounted,ten-inch steel monitoring well box will be utilized to cover the well casing.Drilling activities <br /> will be completed in one(1)day.The property owner will be notified at least one week prior to performance <br /> of any field operations;field work for well installation will not commence until time and date are approved <br /> by property owner. Thereafter, AGE will collect ground water samples on a quarterly basis (for <br /> approximately 2-3 years)from the above referenced well,and will continue to do so until no further ground <br /> water investigation is required by the San Joaquin County Environmental Health Department. <br /> All costs for performance of the subsurface investigation/remediation will be the sole responsibility of <br /> PACIFIC CAR WASH(Mr.Jerry Castro).PACIFIC CAR WASH will be solely responsible for all liability, <br /> costs and expenses for investigation and/or remediation of impacted ground water associated with the above <br /> referenced ground water extraction well.All work at the site will be performed by AGE or subcontractor's <br /> on behalf of PACIFIC CAR WASH. <br /> Upon completion of assessment/remediation,the area investigated on the MANASSERO PROPERTY will <br /> be returned to its original conditions prior to field work activities. <br /> Upon request,the property owners of the MANASSERO PROPERTY will be provided with copies of all <br /> testing and reporting documents for all investigation performed on the MANASERRO PROPERTY. <br /> Furthermore, owners of the MANASSERO PROPERTY will be provided with a copy of the"No Further <br /> Action"letter from the appropriate regulatory agencies upon completion ofthe investigation and remediation. <br /> This agreement for Right-of--Entry is made thisj day of d4�0aR. 2005,by and between <br /> representatives of the PACIFIC CAR WASH(Mr. Jerry Castro)and the MANASSERO PROPERTY(Mr. <br /> Mike Manassero).The undersigned represent and warrant they are duly authorized to execute the agreement <br /> and to bind <br /> the parties <br /> Mr. erry tro e Mr. ike Man sero <br /> R presenting: Representing: <br /> r ^^fiA MANASSERO PROEPRTY <br /> PACIFIC CAR WASH ( !'
