Fes;> <br /> ADDENDUM TO CONTRACT FOR CONSULTING SERVICES t <br /> REGULATORY TECHNICAL ASSISTANCE, <br /> PROJECT PLANNING AND REGULATORY AGENCY NEGOTIATIONS, <br /> CONTINUED QUARTERLY GROUNDWATER MONITORING, <br /> SOIL VAPOR EXTRACTION (SAVE) SYSTEM EVALUATION AND <br /> CONTINUED SAVE SYSTEM OPERATIONS AND MANTAINANCE <br /> Canepa's Car Wash <br /> 6230 Pacific Avenue <br /> Stockton, California <br /> Prepared by: <br /> CONDOR EARTH TECHNOLOGIES, INC. <br /> CONDOR PROJECT NO. 1707 <br /> February 21, 2001 <br /> This Contract Addendum is for regulatory technical assistance (pre-approval request preparation), project <br /> planning and regulatory agency negotiation, continued quarterly groundwater monitoring, SVE system <br /> evaluation and continued SAVE system operations and maintenance at the Canepa's Car Wash located at <br /> 6230 Pacific Ave, Stockton, California (site). This Contract Addendum has been prepared by Condor <br /> Earth Technologies, Inc. (Cc:dor) at the request of Mr. Remo Canepa. <br /> San Joaquin County Public Health Services Environmental Health Division (SJC PHS/EHD) has <br /> indicated that quarterly groundwater monitoring of the eight existing site monitor wells and one of the <br /> nine existing soil vapor extraction wells (alternating) continue. The SVE system was removed from <br /> service on February 6, 2001 to meet the conditions of the Permit to Operate issued from the San Joaquin <br /> Valley Air Pollution Control District (SJVAPCD). The permit requires that the system be shutdown if <br /> destructive efficiency of the SAVE drops below 95% . Field measurements on January 24, 2001 indicated <br /> that the SAVE was operating at 68% efficiency. Despite attempts to tune the system for improved <br /> performance, additional site visits during the next two weeks indicated that the SVE was operating at <br /> 80% efficiency. The system, therefore, has been removed from service until reevaluation of its operation <br /> and efficiency can be conducted. <br /> The undersigned, in consideration of mutual promises herein, hereby agree to the following work to be <br /> performed as an Addendum to the. Contract for Consulting Services between Canepa's Car Wash and <br /> Condor, dated May 2, 1996. The work to be completed by Condor for the above referenced site will <br /> consist of the following tasks: <br /> SCOPE OF WORK <br /> TASK 1 — Regulatory Technical Assistance (Pre-Approval Request Preparation) <br /> • Condor will prepare a pre-approval request to be submitted to the-State of California <br /> Water Resources Control Board, Division of Clean Water Programs, Underground <br /> Storage Tank Cleanup Fund Program for the costs associated with Tasks 1 through 5 of <br /> this Contract Addendum. <br />
