Contingent on agreement with the location of the
proposed boring, the work proposed in the work plan dated May 26, 1998, with the amendments
agreed to in the July 24, 1998, letter is approved. Please submit a PHSIEHD well permit
application, an $89 permit fee and a copy of the encroachment permit from the City of Stockton.
After the well permit has been issued the work can be scheduled with PHSIEHD staff so the work
can be witnessed.
Please be advised that this phase of work may not complete the soil and water investigation and
that preparation of a corrective action plan using limited information is not appropriate.
If you have any questions contact me at(209)468-3449.
Donna Heran, REHS, Director
Environmental Health Division
Margaret Lagodo, Supervising REHS
Site Mitigation Unit
c: CVRWQCB- Mark List
ARCADIS Geraghty & Miller-Dr. Stephen Cullen
Helen McCrary
SWRCB -Jim Munch
SJCAOA- David Irey
A Division of San Joaquin Counry Healch Care Services
