This office <br /> has not yet received the proposal A proposal for this assessment shall <br /> be submitted to this office and the Central Valley Regional Water Quality <br /> Control Board within 10 days of the -date of this letter. Please contact <br /> this office as soon as possible regarding this matter. <br /> } <br /> t <br /> Pursuant to the San Joaquin Local Health District fee schedule Resolution <br /> r <br /> No. 85-58, a $35.00 consultation fee shall be submitted along with the <br /> completed, general Environmental Health Application form for staff review, <br /> of the assessment workplan being required by this office. <br /> Each additional hour over the initial. one hour of time spent in review <br /> activity will be billed at the rate . of $35.00 per hour.
