To proceed with closure of this site, please submit to the EHD a well destruction <br /> permit application along with the required $122.00 per well permit fee, $366.00 inspection <br /> fee and a Master file Record form. Please note that wells located offsite require a separate <br /> permit and fees per parcel of land, along with applicable access agreements or <br /> encroachment permits. <br /> Please provide the EHD certification in writing that all record fee title owners have been <br /> notified of your request for closure pursuant to section 25297.15(a) of the Health and Safety <br /> Code.
