San Joaquin County DIRECTOR
°"'" Environmental Health Den
• Donna Heran,REHS
Department M ASSISTANT DIRECTOR
r. 600 East Main Street Laurie Cotulla, REHS
Stockton, California 95202-3029 PROGRAM COORDINATORS
.'• Mike Huggins,REHS, RDi
Margaret Lagorio,REHS
cqC,fi6w4`P Website: www_sjgov.org/ehd Robert McClellon,REHS
Phone: (209) 468-3420 .teff Carruesco,REHS,RDI
Fax: (209)464-0138 Kasey Foley,REHS
August 14, 2009
Arthur L Wyatt Jr.
6265 Crooked Stick
Stockton CA 95219
Subject: Former Arco Am/Pm Site Code: 1216
4511 Pacific Avenue
Stockton CA, 95207
The San Joaquin County Environmental Health Department (EHD) has received
concurrence from the Central Valley Regional Water Quality Control Board on a
determination that "no further action is required" for the above referenced contaminated
underground storage tank site. The EHD has reviewed the Work Plan for Well
Destruction (WP) dated August 10, 2009, submitted on your behalf by Advanced
GeoEnvironmental, Inc (AGE), and site data for the above subject site and has the
following comments.
In the WP, AGE proposed to over-drill the well with significant groundwater impact, soil
vapor well VW-1, and pressure grout the remaining wells. The EHD concurs with AGE's
proposal to over-drill VW-1. However, some of the remaining wells are located in an area
of either soil or ground water contamination which would require the well to be destroyed
by method of over-drilling in accordance with the San Joaquin County Well Standards
(Weil Standards). Any of the remaining wells that are located in an area of no soil or
ground water contamination may be pressure grouted in accordance with the Well
Standards. Upon review of site data, wells that should be over-drilled include MW1
though MW-9, MW-11 A and B, MW-12 A and B, MW-15, VW-1 through VW-4, EW-1
and EW-2; and wells that could be pressured grouted include, MW40, MW-13, M#'-14,
MW-16 though MW-18. Please note that it is optional to over-drill all of the wells.
To proceed with closure of this site, please submit to the EHD a well destruction permit
application along with the required $405.00 permit and inspection fees for each parcel of
land on which wells to be destroyed are located, and any applicable access agreements
or encroachment permits.
Please provide the EHD certification in writing that all record fee title owners have been
notified of the intent to close this site pursuant to section 25297.15(a) of the Health and
Safety Code. If you are the sole property owner, please provide a statement of that fact
to the EHD (form enclosed).
The above items should be submitted within 20 days of the date of this letter. Once the
well destruction permit(s) have been issued, the work can be scheduled. Please notify
the EHD at least 48 hours prior to performing the well destructions, so the inspections
can be made.
