I <br /> 1 <br /> Permittee shall send reports on a monthly basis Repoits shall be submitted to the City of Stockton Euvnorimental Control <br /> section no later than the 15th day of the following i-nonth <br />' Reports shall include total montlhly flow, in gallons, discharged to the sewer <br /> All reports required by this permit shall include the following certification statement signed by an authorized iepiesentative <br /> I certify under penalty of law that this document and all attachments were prepared under my direction or supeivnsion in <br /> accordance with a system designed to assure that qualified personnel properly gather and evaluate the irifornhation subnhitted <br /> Based on my inquiry of the person or persons who manage the system or those persons directly responsible for gatheling the <br /> information, the information submitted is, to the best of my knowledge and belief,true,accurate, and complete 1 am awai e <br /> that there are significant penalties foi submnttmg false information, including the possibility of fine and nnprisonninilt foi <br /> knowing violations " <br /> Discharge of all flow to the sanitary sewer shall automatically and unrnediately cease upon the occurrence of a lower <br /> explosive limit meter reading equal to or gieater than 10% Permittee shall sample for total petroleum hydrocarbons, <br /> benzene,toluene,xylene, ethylbenzene, and MTBE immediately after a lower explosive lirmt reading equal to or greater <br /> than 10% These samples shall be in addition to regularly scheduled self-monitoring samples Permittee shall notify the <br /> City of Stockton Environmental Control section prior to resuming discharge to the sanitary sewer <br /> Permittee shall insure that all collection, handling, preservation, and laboratory analysis of samples is performed in <br /> lieaccordance with 40 CFR Part 136(copy attached), unless otherwise specified in the monitoring conditions of this permit <br /> Permittee shall perform a quality assurance program,as specified in the City of Stockton,Department of Mumcipal Utilities, <br /> "Quality Assurance Manual for Self-Monitored Industries" (copy attached) <br /> Permittee shall have all flow and lower explosive limit monitori_ug equipment calibrated and certified by a lhcensed techiucian <br /> prior to initial discharge and then at a minimum frequency of once per six months All documentation relating to monitoring <br /> equipment calibration and certification shall be included lin the self-monitoring report for the month in which it was <br /> performed <br /> Peimnittee shall not tamper with, obstruct, disconnect, or bypass any mowtormg equipment used to determine compliance <br /> with any condition contained within this peimit Permittee shall report to the City of Stockton Environmental Control <br /> section within 24 hours of becoming aware of any interruption or malfinIction of momtoimg equipment requned by thus <br /> peimnit <br /> Permittee shall report all permit violations to the City of Stockton Envu onmental Control section within 24 hours of <br /> becoming aware of such violations <br /> Page 2 of 2 <br />
