Secondary containment testing last performedNext due by2011. Submit a 48 hour <br /> test notification of the secondary containment testing and submit its test results within 30 days of <br /> testing to SJC EHD. <br /> 4. Technician found the 89 product line leak detector (LLD) minor leaking from the turbine due to <br /> insufficient amount of thread sealant/doping material. 89 LLD was not able to be tested until the <br /> leak from the LLD on the turbine was stopped. When technician removed the VMI 99-LD2000, <br /> the LLD came out of the turbine easily with a single untwist motion. The LLD was not properly <br /> installed into the turbine snuggly. Technician added more thread sealant/doping material to the <br /> line leak detector and properly placed into the turbine. <br /> 5. Technician found it difficult to remove both 89 and 91 cast iron test plugs in the test port of the <br /> shear valve on Dispenser 13/14. Both test plugs had too much torque placed on. Both test <br /> plugs' threads had been stripped. Technician replaced both test ports with new test plugs. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br />
