UST facility and tank forms for the 87, 89, 91 tank on file with the EHD are not current. Any change of <br /> information on these forms must be submitted to the EHD within 30 days of the changes. Immediately <br /> complete and submit a copy of the facility and tank forms to the EHD within 30 days of receiving this <br /> report. Complete a separate tank form for each tank. <br /> 4. Current financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility within 30 days of receiving this report. <br /> 5. The monitoring and response plans on site are not current and not approved by the EHD. The <br /> monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br /> of the monitoring and response plans for approval by the EHD within 30 days of receiving this report. An <br /> approved copy will be returned for your records. <br /> 7. Alarm logs for 1-3, 1-5, 1-7, L15 - 3/16/11, L12 - 3/28/11, L13 - 3/6/11 were not found on site and also <br /> how each violation were corrected. These records shall be kept on site for at least 3 years. Immediately <br /> locate and maintain the alarm log on site and submit how the violation was corrected. <br /> 17. The 91 turbine I ne leak detector failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, <br /> and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and <br /> obtain a permit withi-i one business day from the EHD. If the failed leak detector can't be replaced <br /> immediately, there is a possibility that the 91 grade UST system may be red tagged to prevent fuel <br /> inputs. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> v/1. -CA! <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
