T — — <br /> 'i <br /> Kennedy/Jenks Consultants <br /> 1.0 INTRODUCTION <br /> This Site Safety and Health Plan, developed in accordance with OSHA standards for <br /> hazardous waste operations (29 CFR 1910.120) and equivalent California CAL-OSHA <br /> regulations (CCR Title 8, Section 5192), establishes general health and safety protocols for <br /> Kennedy/Jenks Consultants (KIJ) personnel at 4555 North Pershing Avenue, Stockton, <br /> California. Addenda containing activity- specific health and safety protocols will be pre- <br /> pared and attached to this Site Safety and Health Pian prior to the initiation of each sub- <br /> stantially different field activity. The Site Safety and Health Pian and activity specific <br /> Addenda, as a minimum, will contain the following information: <br /> (A) Names of key personnel and alternates responsible for site safety and <br /> health and appointment of a Site Safety :Officer. <br /> (8) A safety and health risk evaluation for each site task and operation. <br /> (C) Personal protective equipment to be used 1by employees for each site task <br /> and operations being conducted. <br /> (D) Medical surveillance requirements. <br /> (E) Frequency and types of air monitoring, -personal monitoring and environ- <br /> mental sampling techniques and instrumentation to be used. Methods of <br /> maintenance and calibration of monitoring and .sampling equipment to be <br /> used. <br /> (F) Site control measures. <br /> (G) Decontamination procedures. <br /> (H) Site's standard operating procedures. <br /> (1) An Emergency Response Plan which addresses effective site response to <br /> emergencies. As a minimum, the elements of the Emergency Response <br /> Plan will include the following: <br /> (1) Pre-emergency planning <br /> (2) Personnel roles, lines of authority, training and communication <br /> (3) Emergency recognition and prevention <br /> (4) Safe distances and places of refuge <br /> (5) Site security and control <br /> (6) Evacuation routes and procedures <br /> g:\jobs1940549%H&Sp1an\p1an 1 940549.00 <br />
