Contaminant concentrations continue to be <br /> evidenced in the groundwater at this site. The Report indicated that the groundwater gradient <br /> changed from a northeastern to a northwestern flow direction. If the northwest gradient continues <br /> to be evidenced in the next quarterly monitoring event the domestic wells on the adjoining <br /> properties(3416 and 3516 Newton Rd.)to the northwest should be sampled the following quarter. <br /> Continue quarterly groundwater sampling at this site for TPHg, BTEX, and Oxygenates, until <br /> otherwise directed by PHS/EHD. <br /> PHS/EHD informed you in a letter dated February 23, 2000 that the unused water well on site <br /> located near the southeast corner of the maintenance building should be properly destroyed under <br /> PHS/EHD permit and inspection. Destroy this well under PHS/EHD permit and inspection by <br /> November 15, 2000. Provide PHS/EHD with a well log for this well. If a well log is not <br /> available then you must determine the condition of the water well casing with a down hole video <br /> so that the proper destruction method will be employed. Submit a permit application and$60.00 <br /> fee to PHS/EHD for approval.
