Cummings and Associates (JPCA) is pleased to forward to <br /> you this report for the destruction and abandonment of the three <br /> monitoring wells located on the real property at 18775 Tom Paine <br /> Road, Tracy CA 94576 . <br /> BACKGROUND <br /> This site was given "File Closure" by San Joaquin County Public <br /> Health Services Environmental Health Division (SJCEHD) in a <br /> letter sent to Ernest Costamagna of E G C INVESTMENTS, INC. with <br /> a condition that the three monitoring wells located on the real <br /> property at 18775 Tom Paine Road, Tracy CA 94576, be destroyed in <br /> accordance with SJCEHD regulations. <br /> JPCA acquired the proper permit for the well destruction as <br /> required by SJCEHD. A site specific safety plan was prepared. <br /> FIELD WORK <br /> On September 29, 1999 John P. Cummings met with the SOILS <br /> EXPLORATION SERVICES, INC. a C-57 California Licensed drilling <br /> contractor on site. Michael Infurna, Sr REHS, a regulator from <br /> SJCEHD and Mrs. Gloria Costamagna were on site, also. <br /> The wells were destroyed by removing the stand pipes, pulling out <br /> the PVC well casing, drilling out the well boring and pressure <br /> grouting the wells, as required by SJCEHD. The cuttings were <br /> placed on visqueen. <br /> On October 1, 1999, John p. Cummings and Michael Duzgaj of JPCA <br /> returned to the site and collected a sample of the cuttings in a <br /> clean brass liner tube, capped, labelled and sealed the tube. A <br /> sample of purge water from a drum on site was collected in two, <br /> laboratory certified 40 ml pre-cleaned glass VOAS, with Teflon <br /> 1 <br />
