Cummings&Associates May 6, 1999 File Closure Report" and the June 20, 1999 <br /> addendum and has concluded that no further investigation or remediation is required. In order to <br /> provide a "No Further Investigation" document,the disposition of the existing monitoring wells still on <br /> site will need to be resolved. <br /> Under current San Joaquin County Developmental Codes,wells (monitoring) shall serve their intended <br /> purpose or be properly destroyed under a permit issued by PHS-EHD. <br /> In order to issue the "No Further Investigation" document,you will need to properly destroy the three- <br /> (3) onsite monitoring wells under a permit issued by PHS-EHD. A permit application and$60 fee <br /> should be submitted along with a short workplan that describes the method,materials, and procedures <br /> proposed to destroy the well. After the permit is issued,PHS-EHD will witness the destruction <br /> procedures to verify that the well destruction procedures have been met. <br /> If you would rather not destroy the monitoring wells,you must provide a monitoring and sampling <br /> schedule to PHS-EHD for the three- (3) onsite monitoring wells and the dates that the report of findings <br /> will be submitted to PHS-EHD. Once approved and a continued billing parry established,this program <br /> will be enacted.
