Attending the meeting were <br /> staff members representing Advanced Ge oEnviron mental, Inc. (AGE) and staff <br /> members representing San Joaquin County Public Health Services, Environmental <br /> Health Division (PHS/EHD). <br /> In a telephone conversation on September 1, 1999 with AGE staff, the work proposed <br /> in Site Assessment Work Plan Addendum (the work plan) dated July 28, 1999 (received <br /> by this office on August 6, 1999) prepared by AGE was scheduled to commence on <br /> September 29, 1999. On September 28, 1999, AGE conveyed in a telephone <br /> conversation that the work could not begin on the following day because the <br /> responsible party had not obtained the pre-approval from the State Water Resources <br /> Control Board Tank Cleanup Fund (Fund) for the costs of the work. PHS/EHD <br /> agreed to extend the date to commence the proposed work to October 20, 1999. In a <br /> telephone conversation on October 18, 1999, AGE expressed that they could not begin <br /> work on the rescheduled date. Apparently the application for pre-approval of the costs <br /> of the work from the Fund was not mailed out until mid October. <br /> On November 8, 1999, PHS/EHD staff discovered Monitoring Well #5 (MW-5) was <br /> being installed at the above-referenced site. MW-5 was not a part of the work plan. <br /> PHS/EHD had no prior knowledge of or given approval of its installation. <br /> On November 10, 1999 in a telephone conversation with AGE, PHS/EHD discovered <br /> that an acetate sleeve was not utilized for the continuous core boring of MW-6.
