Information on the Chain-of- <br /> Custody consists of the following: <br /> • Project name and number <br /> • Chain-of-Custody serial number <br /> • Project location <br /> • Sample numbers <br /> • Sampler/recorder's signature <br /> • Date and time of collection of each sample <br /> • Sample type <br /> • Analyses requested <br /> • Name of person receiving the sample <br /> • Date of receipt of sample <br /> • Name, address, and telephone number of laboratory <br /> Sample coolers will be sealed in the field with the completed Chain-of-Custody document <br /> inside the cooler and the express shippers (e.g., Federal Express) shipment forms filled out in <br /> the field and attached to the cooler at that time. The Chain-of-Custody record will have the <br /> signature of the relinquishing field geologist and the shipper's form document tracking <br /> number written on the comment line. <br /> The completed Chain-of-Custody form will be placed in a plastic bag and taped to the <br /> underside of the cooler lid. The cooler will be tightly bound with filament tape. Custody <br /> seals will be signed by the individual relinquishing custody and affixed in such a way that the <br /> cooler cannot be opened without breaking the seals. <br /> 3.5.6 Quality Control Samples <br /> Quality Control samples are collected and analyzed for the purpose of assessing the quality of <br /> the sampling effort and the analytical data. Quality Control samples include trip blanks, <br /> equipment blanks, and field duplicates. The type, description,preparation and collection, and <br /> frequency of Quality Control samples are described below. <br /> Trip Blank <br /> A trip blank is Type II reagent grade water, or better,that is kept with the field sample <br /> containers from the time they leave the laboratory until the time they are returned to the <br /> laboratory. The purpose of the trip blank is to evaluate possible cross-contamination of <br /> aqueous samples during transit or sample collection.
