1.0 HEALTH AND SAFETY PROGRAM <br /> iGeo-Phase Environmental Inc. proposes the following health and safety precautions, which are appropriate <br /> working on sites where workers may come in contact with hazardous waste or hazardous materials. <br /> 1. Emergency Contacts <br /> The following is a list of emergency numbers and health agencies to be contacted in the event of an emergency: <br /> tLocal County Environmental Agency: (209)-468-3442 <br /> San Joaquin County- Environmental Health Dept. <br /> ' Company/Site Health and Safety Officer: (209)569-0293 <br /> Steve Lankford-Geo-Phase Environmental Inc. <br /> Local Fire Department: 911 <br /> ' Stockton Fire Department <br /> Nearby Hospital: (209)944-5550 <br /> Dameron Hospital 525 W. Acacia <br /> The route to the hospital is as follows: <br /> 1. Proceed south on Thornton to its intersection with Hammer Lane. <br /> 2. Turn right(west)on Hammer and proceed 1 1/2 miles to I-5 <br /> ' 3. Turn south on I-5 and proceed about 5 miles to the Pershing Ave.exit <br /> 4. Tank the exit and proceed north '/4 mile to Acacia <br /> 5. Turn right on Acacia 1/2 mile to the hospital on the right. <br /> II. Workers Health and Safety Protocols <br /> t Work Space Safety <br /> The perimeter of the work space should, if possible, be 20 feet from the drilling activity. All work performed in <br /> publicly accessible areas will have a working perimeter to be marked by brightly colored delineators. No <br /> unauthorized personnel will be allowed access into the work space. The project supervisor will coordinate all <br /> ' field activities and maintain the safest possible working environment at all times. If an emergency occurs, the <br /> supervisor will immediately contact the proper authorities(listed above) <br /> Skin and Head Protection <br /> Disposable suits, boots, and gloves will be worn when the working conditions require such precautions. <br /> Disposable Nitrile gloves will be worn at all times during soil and groundwater sampling. Used disposable <br /> clothing will be properly discarded at the end of each working day. <br /> ' Respiratory Protection <br /> Half face respirators will be available for use should the situation warrant their use.
