Louise Avenue <br /> Lathrop, California <br /> Dear Mr. Shih: <br /> This letter responds to the Program Inspection Report by the San Joaquin County <br /> Environmental Health Department(EHD), October 15, 2004 for the ARCO facility <br /> identified above. <br /> 1. Alleged Violation: The UST Program Inspection Report stated, "Annual leak <br /> detector test indicates 91 & 89 leak detector failed. Technician was able to replace on site <br /> with 91 (LD-2000 04081012) 89 (04081013. LD2000). A permit shall be received by <br /> this office next business day." In the corresponding Violation checklist, a check was <br /> placed next to"CCR 2638(a)—Annual line leak detector test(M1)." <br /> Response: In response to your request, we applied for a permit to alter the monitoring <br /> system on October 15,2004. We received approval from your office on October 19, <br /> 2004. The 91 & 89 line leak detectors, model LD 2000 and serial#'s (04081012 & <br /> 04081013), were installed and tested on October 15, 2004. <br /> 1 have enclosed a copy of our application for a permit and your approval. In addition, I <br /> am enclosing the annual Monitoring System Certification, line leak detector test data <br /> sheet and spill/overfill containment box test data sheets. With the transmittal of this letter, <br /> it is our understanding that the above-mentioned alleged violation associated with this <br /> LA2:739760.1 <br />
