APPENDIX 5 <br /> HEALTH AND SAFETY PLAN <br /> LUSTRE-CAL NAMEPLATE CORP . <br /> LODI, CALIFORNIA <br /> 1.0 PURPOSE <br /> This Site Health and Safety Plan (HASP) outlines the health and safety procedures that shall be followed <br /> during field work conducted at the Lustre-Cal Nameplate Corporation (LNC) site (the Site). The <br /> observance and practice of the procedures in this plan are mandatory for all Consultant, Contractors <br /> and Subcontractor employees at the Site. All subcontractors shall be made aware of the requirements <br /> of this plan; however, subcontractors are responsible for the health and safety of their own <br /> employees and for following all applicable federal, state,and local regulations. <br /> This plan has been reviewed by the Project Manager and Project Health and Safety Officer. Prior to <br /> entering the Site, Consultant and Contractor personnel shall read this plan and be familiar with <br /> health and safety procedures required when working on Site.
