Appendix 5 <br /> Health and Safety Plan <br /> Lustre-Cal Nameplate Corp. <br /> 5.2 POTENTIAL PHYSICAL HAZARDS AT SITE <br /> Potential physical hazards, as those listed in Table 5.0, are discussed below. <br /> 5.2.1 Underground Utility Hazards <br /> An underground utility check shall be performed prior to initiating any subsurface investigation or work. <br /> The check will include: <br /> X USA Note: USA must be notified at least 2 working days before any subsurface work <br /> begins. Record confirmation number in project field notes. <br /> X Private Locator: Consultant <br /> X Plans Check. Facility Contact: City of Lodi <br /> x Geophysical Survey. Consultant <br /> Additional Information: Much of the proposed work will take place in public right-of-ways. Extreme <br /> care to identify underground utility hazards will be taken. The initial 5 feet of each boring will be advanced <br /> or probed by hand before using power equipment. <br /> 5.2.2 Overhead Power Lines <br /> Whenever possible, avoid working under overhead high voltage lines. As a general rule,hearing protection should be worn when two people standing <br /> within 2 feet of each other cannot communicate at normal conversational voice levels. <br /> 5.2.4 Heat Stress Hazards <br /> Heat stress is a major hazard, especially for workers wearing protective clothing.
