• Inclusive dates and times of possession <br /> • Air Bus Bill Number <br /> To maintain chain-of-custody, each person in custody of the sample will sign, date, and note time <br /> on the form. Samples will not be left unattended unless placed in a sealed container secured with <br /> custody seals, with the chain-of-custody record inside the container. <br /> 9.5.2 Custody Seals <br /> Custody seals are used to detect unauthorized tampering of samples following sample collection, <br /> up to the time of analysis. The seal will be attached so that it must be broken to open the sample <br /> shipping container. Seals will be affixed to each shipping container(i.e., each ice chest)before <br /> the samples leave the custody of the sample personnel. A seal will include the following <br /> information: <br /> • Sampler's signature <br /> • Date of collection <br /> 9.5.3 Field Log Book <br /> All information pertinent to the field sampling effort will be recorded in a log book or equivalent <br /> standardized form. Each page/form will be consecutively numbered. All entries will be made in <br /> indelible ink and all corrections will consist of line-out deletions that are initialed and dated. <br /> Entries in a log book may include the following: <br /> • Purpose of sampling <br /> • Location and description of the sampling point <br /> • Details of the sampling site (e.g., distance from a permanent feature) <br /> • Name and address of field contact <br /> • Documentation of procedures for preparation of reagent or supplies which becomes an <br /> integral part of the sample (e.g., field blanks) <br /> • Identification of sampling crew members <br /> M ain\D:AEnvironmenta1\LN CV\10152007.wpd 64 <br />
