Final Monitoring System Certification, Leak Detector and Spill Bucket testing prior to <br /> facility approved to operate. Testing started 03/14/11. Completed on 03/15/11. <br /> On 3/14/11- Conducted monitoring system certification - verified positive shutdown, failed safe, sensor <br /> out. Conducted leak detector testing - able to detect 3 gallons/ hr @ 10 psi. conduct 91 and diesel spill <br /> bucket testing - verified 5 gallon capacity. <br /> On 3/15/11 - 87 spill bucket testing passed - verified 5 gallon capacity for both buckets. Verified <br /> functionality of all spill buckets drain valves. 87 spill bucket was not conducted on 03/14/11 since its rain <br /> guard lid was not installed. <br /> 5JC EHD received a Monitoring Plan. Information is not correct. <br /> Facility can be approved to operate once SJC EHD received the revised corrected Monitoring Plan. <br /> Notes: <br /> - Received Certification of Installation (Form C) for Tank, Piping, Sump and D;spenser Installations. <br /> Dated 03/11/11 and signed by Contractor of Gettler-Ryan. <br /> - Received As-Built Site Plan. Dated 03/10/11 from Gettler-Ryan. <br /> - Received Enhanced Leak Detection test results. Test start date 03/08/11 - all system components <br /> passed. Signed by Leak Detection Technologies field technician dated 03/09/11. <br /> - Received financial responsibility dated 11/10/10. Insurance by ACE American Insurance Company <br /> coverage period 01/01/11 - 01/01/12. <br /> - Received Facility Form A, Tank Forms B, Response Plan. <br /> - Immediately contact Designated Operator to begin employee training within 30 days of operating new <br /> UST operations systems - maintain records onsite. Begin monthly visual inspections starting this month. <br /> - Reviewed facility's BP Best Management Practice Training Guidelines/ Emergency Response <br /> Procedures. Topics includes: Emergency contact & phone numbers, Fire, minor spill response (<5 <br /> gallons), major spill response (>5 gallons), UST monitoring procedures & alarms, carbon dioxide <br /> awareness, waste storage.
