The sample label (see Attachment 11) on each sample container <br /> should be completed in its entirety to assure that all pertinent information <br /> accompanies the sample to the laboratory. Complete information on each label <br /> is necessary so that if the sample container is separated from the rest of the <br /> shipment in the laboratory, sufficient information is available on the Jabal <br /> to ascertain the scheduled work required on the sample. The chain-of-custody <br /> form should reflect the identification of each sample for which laboratory <br /> results are requested. If samples are to be held at the laboratory pending <br /> further instructions, an appropriate comment should be made on the chain-of- <br /> ', custody form. Each sample must be uniquely identified. The administrative <br /> sample tracking system maintained by the laboratory allows each sample batch <br /> to be identified with a alpha-numeric label containing a maximum of 20 <br /> characters. <br /> B. Sample Receipt <br /> Sample receipt is the first contact that the laboratory has with the sample. <br /> Control and documentation of all samples are instituted at the time of <br /> receipt. Upon receipt of the samples to the laboratory, the Sample Custodian <br /> will verify that the incoming samples correspond to the chain-of-custody and <br /> note the.condition in which the sample is found. Missing samples, samples <br /> which have broken, or become compromised during shipment, are noted on the <br /> chain-of-custody form. If no chain-of-custody form is found in the shipping <br /> container, an inventory of the samples is taken and the person from whom the <br /> samples were received is immediately contacted. A chain-of-custody form is <br /> then completed by the sample custodian in accordance with the instructions of <br /> the sampler/client, After checking the chain-of-custody form against the <br /> samples received, the sample custodian then obtains a Lab Project (LP) number <br /> from the data control section and assigns the LP number to the chain-cf- <br /> custody. <br />
