(Submit a signed EHD Master File Record-
MFR will do this)
if you can assure me all of these conditions can be met, I can charge you
about half of the up front permit fees (14 x $122) for the permit(s).
I'm assuming there are some ONSITE wells and OFFSITE wells needing
repair? correct? if so, I will have to issue separate permits. (on versus
offsite). Each needs it's own fee. I'm thinking four ONSITE and four offsite
wells for EHD fee purposes. This will provide roughly half of the $1708 fees.
So submit the work plan (#3 above) and the details of your proposal and a
permit for onsite with $488 and the same for offsite.
attached is our webpage with the forms you'll need to submit in PDF format.
print each out. (2 page permit application & MFR and an "info" sheet). read
the info sheet. Complete the permit applications and send each in with $488.
ALSO, complete, sign and also send in the MFR. this identifies WHO will pay
the EHD for any charges OVER the $976 you send in.
http://www.sjgov.org/ehd/Programs/Environmental/Borin.gs.htm
Only thing that may change this all is IF the City of Tracy is not involved, as in
NO well repairs in the City right of way. then just send in ONE permit
application, $976 and a completed-signed MFR.
call me if you have additional questions...
mike
209-468-3454.
From: Gehlke, Sean A. The wells were all constructed in the mid-1980's with electric box or water box
covers. These will be replaced with standard monitoring well boxes. In addition, the
top of the MW-6 PVC casing is cracked, and will be repaired. We expect to complete
the repair/replacements in July or August.
I wanted to confirm that no permits will be required from Environmental Health for
this scope of work, as these are all surface repairs and replacements.
Thanks,
9/15/2011
