SAN J OA U I Environmental Health Department <br /> COUNTY <br /> — <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: <br /> TFacility Address: Date: <br /> TIKI LAGUN RESORT& MARINA 88 W MCDONALD RD, STOCKTON May 12, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 10 HSC 25508.1(a)-(f) Failed to submit revisions to business plan within 30 days of substantial change in operations. <br /> During the inspection, Nineteen 5 gallon pails of Henry 784 Aqua-Bloc, sixteen 5 gallon pails of sodium hydroxide, <br /> one 261 cubic foot cylinder of compressed hydrogen gas were found on site that have not been reported and <br /> greater than 100% increase in the amount of carbon dioxide gas was found on site and the updated information has <br /> not been submitted. Within 30 days of one the following events, business shall electronically update the information <br /> submitted to the California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Documentation showing initial and annual refresher training was not available at the time of inspection. The <br /> business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher <br /> training. All employees shall be trained in safety procedures in the event of a release or threatened release of a <br /> hazardous material, including, but not limited to, familiarity with the following plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
