QUALITY ASSURANCE AND QUALITY CONTROL PLAN <br />The Quality Assurance/Quality Control Plan (QAQCP) is intended to facilitate the acquisition of accurate and <br />reliable data. Environmental data gathered during the shallow soil and soil vapor sample collection work will <br />be collected and analyzed following procedures prescribed in the Quality Control Plan. A Quality Assurance <br />Plan has been established to assure that the Quality Control Plan is effective. As required all samples <br />collected during the scope of work will be submitted to California Department of Health Services (DHS) <br />Environmental Laboratory Accreditation Program (FLAP) certified laboratories for analysis following <br />ASTM D 4840 chain of custody procedures. The following field and laboratory procedures will be <br />implemented to ensure that QAQCP objectives are met. <br />1.0 FIELD DOCUMENTATION AND INVESTIGATION REPORTING <br />All information pertinent to the scope of work will be kept in a field log book/daily log. All field <br />documentation forms will be recorded in ink and maintained for five years following the conclusion of the <br />project. No documentation will be destroyed or discarded. If mistakes or errors are made on any field <br />documentation form, the error will be crossed out using a single line through the incorrect data leaving the <br />mistaken entry legible and the correction will be initialed and dated. All analytical data will be reviewed and <br />validated for quality control by the accredited laboratory and Ground Zero Analysis, Inc. (Ground Zero). A <br />report of findings and analytical testing will be prepared by Ground Zero following the completion of the soil <br />and soil vapor sample collection. Information to be documented and reported includes at least the following: <br />- Sample numbers <br />- Locations of sample collection <br />- Soil boring or well numbers, as applicable <br />- Depths at which samples were obtained <br />- Names of collectors <br />- Dates and times of collection <br />- Purpose of sample <br />- Sample distribution (e.g. laboratory, archive, etc.) <br />- Field observations <br />- Field measurements (e.g. PID readings, pH, conductivity, water levels.) <br />- Other data records (e.g. development log, soil sampling report, well log, etc.) <br />2.0 LABORATORY ANALYTICAL METHODS <br />The primary analytical laboratory will be: <br />1. BC Labs of Bakersfield, California (FLAP #1186) for the analysis of the soil and soil vapor samples <br />Soil samples will be collected in either 8 -ounce widemouth clear glass jars provided by the laboratory or in <br />clean brass or stainless steel tubes. Screw on caps will be fitted with Teflon liners. Tubes will tightly capped <br />and sealed. Analytical methods, containers, preservatives, and hold times to be used or that may be used <br />during the soil sampling and analysis phase of the scope of work are summarized in Table 1.
