Violation 102: Clarify where the dirty oily mop bucket water is dumped. <br /> 2. Violation 118: Submit a consolidated manifest from the following dates and for the disposal of the following <br /> hazardous wastes: <br /> a. 7/13/2017, 540 gallons of used oil* <br /> b. 9/26/2018, 690 gallons of used oil <br /> C. 11/6/2018, 990 gallons of used oil <br /> d. 1/31/2019, 385 gallons of waste antifreeze <br /> e. 2/28/2019, 32 pounds of waste described as "environmentally hazardous substance" and waste code 451* <br /> f. 2/28/2019, 85 gallons of waste corrosive liquid* <br /> g. 4/22/2019, 24 pounds of waste described as "environmentally hazardous substance" and waste code 451* <br /> h. 4/22/2019, 55 gallons of waste corrosive liquid* <br /> i. 6/20/2019, 24 pounds of waste described as "environmentally hazardous substance" and waste code 451* <br /> j. 6/20/2019, 55 gallons of waste corrosive liquid* <br /> *For items a. and a-j. above: you previously submitted uniform hazardous waste manifests from the company that <br /> consolidated your facility's waste with other facility waste, however you need to submit a consolidated manifest receipt <br /> that shows the specified amount and type of waste noted above was picked up from your facility. <br /> 3. Violation 119: Submit a corrective action statement documenting how the facility has ensured that the <br /> consolidated manifests, specifically those from Heritage Crystal Clean LLC, will contain the following: <br /> a. A legible contact person name of the generator <br /> b. A statement signed by the generator certifying that the generator has established a program to reduce the volume <br /> or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be economically <br /> practicable. <br /> Note:the picture of the consolidated manifest you submitted does not have either of the items listed above. See the <br /> clear photo of the same consolidate manifest, attached,for your reference. <br /> 4. Violation 301: Submit a corrective action statement and supporting documentation that demonstrates how the <br /> spill clean-up waste from the Dyna Clean Parts Cleaning System spill was disposed of. <br /> 5. Violation 406: Submit a corrective action statement and supporting documentation that demonstrates how empty <br /> drums that remain on-site for longer than one year are discarded. <br /> 6. Violations 503 and 512:The violations will remain open until the complete tank assessment for the 1,000-gallon <br /> used oil tank and two 275-gallon used antifreeze tanks are received. <br /> 7. Violation 602: Submit a copy of the hazardous waste disposal record for the 55-gallon satellite drum of oily solids <br /> observed in the D&A area for longer than one year. <br /> 8. Violation 607: Submit a corrective action statement documenting that damaged lead acid batteries will be labeled <br /> with the complete initial date of accumulation (i.e. month, day, and year). <br /> Note: Manifest 017461026JJK, which you stated you could not find for violations 114 and 115, is attached. <br /> 4 <br />
