Generate a training record with a list of trained employees and the date of training. <br /> Provide a corrective action statement, the training record, a detailed description of the training, and supporting <br /> documentation to the San Joaquin County Environmental Health Department(EHD)within 30 days. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Notes: <br /> -The inspection report has been amended by adding violations 103, 118, 119, 406, and 602, and removing <br /> violations 601 and 708. <br /> - The inspection began on 2/13/2020 and was finished on 2/14/2020. On 2/14/2020 consent to conduct the <br /> inspection and take photos was granted by Johnaton Jasso, Service Manager. <br /> - On 2/14/2020, at least ten or more waste fluorescent lamps were found not containerized in a closet near the <br /> south-west corner of the Warranty Shop indicated on the facility's site map. According to Doug Martin of <br /> Facilities Maintenance, the waste fluorescent lamps have been stored on-site for over a year. Immediately <br /> properly manage the universal waste fluorescent lamps in compliance with Title 22 California Code of <br /> Regulations (CCR) Submit a corrective action statement and supporting documentation to the EHD within 30 <br /> days. <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> amended and finished on 2/26/2020 and replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 3/27/2020. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
