540 West Scotts Avenue • SeJ*mber 8, 1998
Project 9644 Page 2
4. The boring holes were then measured and it was determined that
there was less than 10% cave in in any of the holes and the total
depth of each boring was about 40 feet. Approximately 10 feet of
water remained in hollow stem auger.
5. Neat cement grout (five sacks of cement to approximately 20 gallons
of water) was then pumped into the bottom of each of the drilled out
wells using the hollow stem auger as a casing. The auger , was
withdrawn as the grout was placed in such a manner as to prevent
voids from occurring. After all the auger was removed, each boring
was topped off. The site was then finished off to match the adjacent
sidewalk.
6. The soil, casing and vault from each of the wells were then disposed
of as construction debris.
As required by the terms of the State of California Department of Water Resources,
Form 188 for the monitoring well destruction has been completed and filed as
required as well as attached to this letter.
If there an any leage call.
14
Sincerely,
v
Q N0.23772 m
EXP. 12-31-99 "
Jo Carve sol OvIL
C1vi Engine UFPV}i0IJi3:I
S
i ovsky No.REA•2540G`
Pre dent Expires 6/30/-1y
WILLIAM DUBOVSKY ENVIRONMENTAL
