Here is what I need to <br /> close out those violations (I've included the corrected by dates since some have not come yet): <br /> #102-(Completion date provided: 2/28/2020) Please provide a copy of the transformer oil analytical & hazardous waste <br /> determination <br /> -What happened to the refrigerant that was removed from the mini-fridge? <br /> -Please provide the analytical & hazardous waste determination for the metal fines <br /> #104-(Completion date provided: 4/30/2020) Please provide a copy of the source reduction plan <br /> #118-(Completion date provided: 3/31/2020) Please provide a roster and syllabus demonstrating that relevant <br /> hazardous waste training has been received by all applicable employees <br /> #208-(Completion date provided: 3/31/2020) Please provide a roster and syllabus demonstrating that individuals <br /> performing treatment in the lab have been properly trained <br /> #301-(Completion date provided: 12/31/2019) Please provide a statement and/or supporting documentation that the <br /> milling machine is not leaking oil anymore <br /> #303-(Completion date provided: none) Were the eyewash bottles replaced yet? <br /> #709-(Completion date provided: 4/30/2020) Please provide a roster and syllabus demonstrating that individuals who <br /> handle universal waste are properly trained <br /> If you have any questions or need assistance please let me know. One other thing I noticed. In the RTC it said the <br /> refrigerant was removed from the mini-fridge. Refrigerant could be considered a Material that Requires Special Handling <br /> and only Certified Appliance Recyclers can remove those items from appliances.
